As you prepare for a visit to The CORE Institute Specialty Hospital, rest assured that most of your basic needs have already been taken care of, including such items as food, hospital gowns, and toiletries. Here you will find a short list of essentials that you should plan to bring with you, as well as a list of items you should leave at home.

What to Bring to the Hospital


  • Please bring a complete list of all medications you are currently taking, which should include any over-the counter medications and herbal supplements.
  • Please bring any Lab and/or XRAY results that may have been done at another facility.
  • Please bring a case for contact lenses, glasses, and/or dentures.
  • Please bring your insurance card.
  • Please bring a Valid ID.
  • Please bring a form of payment.
  • Please bring a copy of your Advanced Directives (i.e., Living Will, Power of Attorney).
  • If staying overnight, please pack sleepwear, footwear, and other personal items you may need.

What NOT to Bring to the Hospital


  • Please do NOT bring valuables such as jewelry, wedding rings, or money.
  • Please do NOT bring electrical appliances such as Cell Phones, TV’s, and Radios.
  • Please do NOT bring cigarettes, alcohol, or illegal drugs.

Advanced Directives

All patients have the right to decide which treatments they will accept and which they will refuse. Once your doctor has explained your options, you can put your wishes in writing with an Advanced Directive.

Our staff will follow your directions to ensure your wishes are carried out, so your family doesn’t have to make difficult medical choices on your behalf.

Advanced Directives are documents written in advance of the time when you may be unable to make health decisions for yourself. You have the right to make important legal decisions related to your care, and we can help you through this important process.

There are four types of Advanced Directives recognized under Arizona Law:

  • Directive to Physicians and Family (Living Will)
  • Medical Power of Attorney (POA)
  • Out of Hospital, Do Not Resuscitate (DNR)
  • Declaration of Mental Health Treatment (DMHT)
